چکیده
Objectives: To evaluate the associated risk factors and to determine the impact of management strategies on the outcome of PPHN. Study Design: Prospective descriptive study in tertiary center included 40 neonates having PPHN. All patients received the conventional therapy for PPHN, sildenafil as adjuvant therapy was added in cases of failure. Results: The study included 23 males, 17 females with a mean gestational age 37.25 ± 2.6 weeks. Male patients had significantly higher systolic pulmonary artery pressure (SPAP) and higher mortality rate compared to females [7/23 (30.4%) versus 1/17 (5.9%), p = 0.04]. Infants of diabetic mothers had significantly higher mortality rate (p = 0.003). Components of the blood gases; PH, PCO2, HCO3 improved dramatically after completion of the different lines of treatment (p = 0.001). A statistical significant drop of SPAP after application of the different modalities of treatment (p = 0.001). Addition of sildenafil was effective in the reduction of the duration of NICU stay and SPAP below 40 mmHg and (p = 0.001, p =0.0001, respectively). The overall mortality rate was 8/40 neonates (20%), however, the mortality among the patients who received sildenafil in addition to conventional therapy was 1/14 neonates (7.14%) of those group with p = 0.001).

Prevalence of Persistent Pulmonary Hypertension of the Newborn and Associated Factors in Term Neonates Born in Southeast of Iran, 2020
Background and purpose: Infants with persistent pulmonary hypertension (PPHN) are readmitted to hospital three times more than other infants or die after discharge. The disease imposes a high cost on families and health systems. In this study, for the first time, the prevalence of PPHN and associated factors were investigated in mature neonates born in southeast of Iran. Materials and methods...
متن کاملRecent Trends in the Management of Pulmonary Hypertension
Context. Pulmonary hypertension is a common clinical problem encountered in day-to-day practice. Drug therapy forms the backbone of management and appropriate drug selection is based upon the underlying causes. The options available are widening with availability of newer therapeutic agents and thus it becomes necessary to be updated with the recent developments. Objective. To review the existi...
متن کاملMeconium aspiration syndrome in neonates
Perhaps the most significant changes in airway management over the recent years have been in the management of an infant delivered through meconium- stained amniotic fluid( MSAF). MSAF occurs in approximately 10℅ to 20℅ of all deliveries and increases to over 30℅ in deliveries after 42 weeks gestation. Meconium aspiration syndrome ( MAS) occurs in about 2℅ to 5℅ of these cases with a high morta...

Background: Vein of Galen aneurysm (VGA) is the most common form of symptomatic cerebrovascular malformation in neonates. It develops in a fetus in the first trimester of pregnancy due to unknown reasons, but it is likely to have a genetic etiology. The prognosis of VGA is usually poor, particularly in newborns with heart failure due to high-flow intracerebral shunt. Surgery and endovascular em...
